Hopkins Community Connection acceping applications!
Hopkins Community Connection is taking advocate applications for five clinical sites for the Spring 2023 semester! If you're interested in making a meaningful impact in helping the Baltimore community address the social determinants of health, consider applying! There are no pre-requisites or necessary prior experience, and we are currently looking for fluent English and Spanish speakers! The application will be closing on October 23, at 11:59 pm.

Hopkins Community Connection (HCC) is program within the Johns Hopkins University School of Medicine that supports the social needs of a diverse patient population in order to improve overal health of our patients and communities. Medical providers refer patients who are interested in accessing essential health resources (housing, food, health, insurance, transportation, and more) to HCC. Accepted HCC advocates spend the semester working 6 hours per week creating action plans with patients and providing weekly follow up calls to address any barriers to successfully accessing resources that may come up along the way. In this way, by addressing the social aspect of health as a supplement to a physician’s medical expertise, we hope to offer our patients comprehensive, equitable, and holistic care.
