Our Mission

Hopkins Community Connection collaborates with communities to challenge systemic barriers to health equity by training a diverse workforce of individuals who champion a more just system through an innovative and patient-centered approach to addressing social determinants of health in the healthcare setting.

Our Work

As an undergraduate student volunteer organization, we address essential needs of patients through universal social needs screenings and navigation to community resources and benefit programs. Our organization partners with JHMI and staffs Family Resource Desks at the Harriet Lane Clinic, Comprehensive Care Practice, Bayview Children's Medical Practice, Johns Hopkins Outpatient Center/Levi Watkins Outpatient CenterEast Baltimore Medical Center, and Beacham Center for Geriatric Medicine to address the social drivers of health. We assist Baltimore families to access essential resources such as food, housing, heat and electricity in order to be healthy. Volunteer advocates then follow up with their clients, on a weekly basis, to ensure they get the appropriate resources to address their needs. By learning about the health care system, addressing psychosocial needs, and making lasting bonds with Baltimore families, we will become the nation's next great health care leaders.
